V akutni fazi nekje do 88% pacientov po mo\u017eganski kapi ka\u017ee senzo-motori\u010dno disfunkcijo zgornjih udov in te vztrajajo pri 55% do 75% pacientov ve\u010d kot tri mesece.\u00a0<\/p>\n<\/div><div class=\"fusion-separator fusion-full-width-sep\" style=\"align-self: center;margin-left: auto;margin-right: auto;margin-top:25px;width:100%;\"><\/div><div class=\"fusion-text fusion-text-2\"><h3>Slab\u0161e okrevanje zgornjih udov vodi v omejevanje vsakodnevnih aktivnosti in kvaliteto \u017eivljenja.<\/h3>\n<p>Pri zgornjem udu se razvije vedenjski vzorec preterane uporabnosti ne-hemiplegi\u010dnega uda oziroma fenomen razvoja nau\u010dene neuporabnosti hemiplegi\u010dnega uda. To pomeni, da se osebe NAU\u010cIJO neuporabljati svojo slab\u0161o roko in se NAU\u010cIJO veliko bolj uporabljati svojo bolj\u0161o roko.\u00a0<\/p>\n<p>Tako v akutni kot tudi kroni\u010dni fazi je zelo pomembno, da spodbujamo uporabo slab\u0161e roke. Ko govorimo o roki imamo v mislih podlaket, zapestje, dlan in prste. Seveda, pa so zelo pomembni tudi preostali deli zgornjega uda ter trupa, ki prenesejo roko v prostor proti predmetu. Te vaje lahko najdete na tej povezavi.\u00a0<\/p>\n<h3>Roka je lahko popolnoma neuporabna<\/h3>\n<p>Roka je lahko popolnoma neuporabna, oseba lahko ima spremenjene ob\u010dutke v dlani in prstih od mravljin\u010denja, ne\u010dutenja, bolj izra\u017eenega \u010dutenja podlage\/predmeta ali imajo prisotne bole\u010dine. Prav tako se velikrat pojavlja t.i. spasti\u010dnost ali povi\u0161an mi\u0161i\u010dni tonus v mi\u0161icah podlakti, zapestja, dlani in prstov in\/ali nezmo\u017enost zadostne mi\u0161i\u010dne mo\u010di, da bi odprli roko, prijeli predmet in ga dr\u017eali toliko \u010dasa, da ga lahko smiselno uporabljajo v vsakodnevnih aktivnostih. V mnogih primerih, ko gre za huj\u0161o obliko po\u0161kodbo mo\u017eganovine se pojavijo tudi t.i. pridru\u017eene reakcije\u00a0 kot je spazem v roki, ko oseba zazaha, kihne, zaka\u0161lja, ko hodi, ko poizku\u0161a premakniti zgornji ud v prostor proti predmetu, itd.\u00a0<\/p>\n<p>Roka, ki pa ima ohranjene dolo\u010dene selektivne aktivnosti prijemanja in rokovanja s predmeti pa je lahko okorna, nerodna in prepo\u010dasna, zato je osebe nau\u010dijo jo manj ali pa sploh ne spontano uporabljati.\u00a0<\/p>\n<p>Kar osebe najve\u010dkrat zaznajo je, da roke ne zmorejo samostojno\/hoteno odpreti. Zato velikokrat preterano razstezajo dlan in prste, vle\u010dejo roko na silo in jo poizku\u0161ajo popolnoma odpreti. Tak\u0161na ponavljajo\u010da aktivnost pa lahko vodi do preterane razsteznosti sklepne ovojnice, sklepnih ligamentov in mi\u0161ic ter izgube lokov v dlani. \u010ce oseba izgubi loke v dlani, jih ima preterano razstegnjene, ne bo ve\u010d mogla oblikovati prijema premeta in ga ne bo mogla uporabljati v vsakodnevnih aktivnostih. Vsekakor pa je pomembno, da osebe roko vsakodnevno razgibavajo in ohranjajo pro\u017enost zapestja, dlani in prstov.\u00a0<\/p>\n<p>\u010ce razmi\u0161ljamo o vsakodnevnih aktivnostih opazimo, da roko le redko kdaj popolnoma odpremo. Ve\u010dinoma potrebujemo roko le delno odprto, to\u010dno toliko kot je velik predmet. \u00a0<\/p>\n<p>Slede\u010da te\u017eava, ki se pojavi je, da predmeta, ki so ga prijeli ne morejo izpustiti. Zato pri\u010dnejo roko vle\u010di nazaj, pri tem pa pride do pove\u010danega spazma in prsti \u0161e bolj oprimejo predmet. Da predmet lahko spustimo je najprej potrebno spustiti prijem in ne odpreti roko! Zato v prvi fazi u\u010dimo kliente, da bodisi sprostijo prijem ali pa poizku\u0161ajo drseti po prementu navzdol. Ko to zadostikrat ponovimo, bo spu\u0161\u010danje predmeta mnogo la\u017eje in hitrej\u0161e prav tako bo manj pridru\u017eenih reakcij z ramenskem sklepu in komolcu.\u00a0<\/p>\n<h3>Da roko za\u010dnemo uporabljati v vsakodnevnih aktivnostih potrebujemo vztrajnost in pozornost!<\/h3>\n<p>Velikokrat spontano raj\u0161i uporabimo dominatnej\u0161o, bolj\u0161o roko npr. za odpiranje vrat, potegniti stol stran od mize, odpreti vodno pipo, itd. Zato, da ponovno za\u010dnemo spontano uporabljati slab\u0161o roko je potrebno veliko vztrajnosti, ponavljanja in koncentracije. Da kljub temu, da bi spontano uporabili bolj\u0161o roko, se spomnimo in uporabimo slab\u0161o. V na\u0161i kliniki se s klienti dogovorimo katere aktivnosti bo od sedaj naprej izvajal s slab\u0161o roko. Zato, da bi odprli vrata ali pri\u017egali lu\u010d ne potrebujemo odprte dlani in roke \u2013 to lahko naredimo tudi s pestjo.
